Below is a letter that can be given to patients who are making back injection appointments but don’t have “consult or consultation” written on the referral.
You can verbally let the patient know but this note can also be given as an explanation for what is required.
The letter can be printed from here but will be located in – P Drive, Practice Docs, Practice Staff Docs, Consultation letter.
