Twins

This week it was discovered that two twins were merged as one patient because they shared a date of birth and last name. The Medicare cards, first names and addresses were different yet for some reason someone saw fit to merge the patients. Additionally, it almost happened with another set of twins. Please ensure you are checking all information and confirming with the patient before any merge.
