Mislabelled/Missing Images

There have been an increasing number of studies lately that are mislabelled or missing from PACS. We cannot stress enough the importance of:

• Correct labelling on films for side
• Record keeping of when a patient indicates the referral requests the wrong side (make a note on the referral and rescan, and add notes to Comrad)
• Ensuring images are sent through to PACS
• Ensuring a scan is performed under the correct patient

Additionally, when efforts are made to chase up the above errors they should be considered just as important as the patient about to be scanned. It needs to be remembered that none of our jobs are done until the report and films have been sent to the referrer and patient.
