Medicare Forms

Medicare forms need to be signed by patients once their exam is completed.  There are two types of forms available.  The green (old style printable copy that Medicare provides directly) or the electronic printable form (which will be available to print from the P drive).


Process:


  • Once the patient’s label has been printed, attach one of the labels (the one with the patient’s name, address, medicare number, DOB etc. – medium sized label) on the Medicare form – Please see example A and B Medicare form below.
  • Put the claim form into the patient’s folder so that once the examination is completed the Technical/Sonography staff will ask the patient to sign the claim form. 
  • The signed claim form will need to be scanned into the patient record (under the “Medicare” tab).  Currently this is labelled "Attach extra (4)" but will be renamed by next week to Medicare.  Concierge staff will need to scan this in on behalf of the Sonographers and each other Dept’s Technical or Clerical will be responsible for this.



Every single patient who is being bulk billed needs to sign one of these forms including workers compensation patients (and this is because if their claim is declined, then the only way we will be paid is to process through Medicare).


The forms will need to be put together in an organised daily bundle and then weekly bundle.  The weekly bundle will then need to be forwarded to 105 Main Street – Wcomp staff – Rani and Therese who will make sure that each patient has a signed form.  (PLEASE NOTE THIS DOES NOT MEAN THAT THE STAFF AT EACH LOCATION ISN’T CHECKING THAT THE FORM HAS BEEN SIGNED AND ORGANISING FOR IT TO BE SIGNED IF IT HASN’T BEEN – THEIR JOB IS TO REPORT BACK TO DR LEE TO LET HIM KNOW IF IT’S NOT BEING DONE)!!!   It is the responsibility of the Sonographers and Technical staff to get the forms signed but the clerical staff will provide the support in helping with scanning the documents as needed.

Can each senior from each site please place an order for these so that they come out directly to your site (Rita has put the Dr to put it under next to your name – you will need to document their provider number for your location – if you don’t know it let Rita know). There is a max number you can order – perhaps it’s 500 in a box, so if you put down 3000 and we’ll see if they deliver this – the form is DB4E – Rita has marked it on the order form with an * .  If you run out or Medicare doesn’t deliver then we’ll use the printable version that is in the P drive under Practice Docs, Medicare Forms, Medicare – electronic form.  Point is, we should never be in a situation where we’re told someone hasn’t signed because there are no forms!
