If there is any information that should be recorded for a patient's visit, you must add it to the visit notes. The notes are there to keep everyone informed - remember, many staff members will interact with a patient and their visit record; information is vital and notes save time. Currently too much time is wasted trying to follow up on a visit when the information could have been added to the notes.
Visit notes are not just for admin staff to enter!
If your patient indicates the request has the wrong side - add it to the notes. If the patient needs to come back to complete the test - add it to the notes. If the patient has a reaction to contrast - add it to the notes. If the patient says they are an alien sent by higher beings to teach us all how to live - add it to the notes!
