If a patient sends a photograph of their request form when making an appointment, please ask them to bring in the original form when they arrive for their examination.
When registering the patient on Comrad and a request form has already been uploaded to the visit, delete the request and scan the original. It is imperative that the information included on the request form is legible.
Below is an example of a photograph of a request that was uploaded to ensure we book the patient in for the correct examination. Whilst this is acceptable for those purposes, it is completely unacceptable as a scanned copy when the patient arrives for their visit.
The second example shows the same request form when the original has been scanned.
The difference when zooming in on Comrad is night and day - the first request is completely illegible, the second can actually be read.
