All scanned documents, especially request forms, must be easily read and of good quality. If a patient provides a photo of a request, the original must always be obtained and scanned in. Whilst it is OK for a photo of a referral to be used when making an appointment, it is unacceptable for the actual visit. It is the responsibility of the staff member registering the patient to ensure the original request is scanned in.
