Request Forms

No patient is to be sent away if they don’t bring their request unless all attempts to contact the referrer have been made, or attempts to have the frequest sent by other means have been exhausted e.g. patient may have left request at home and someone at home can SMS or email a copy to the practice.  If all else fails e.g. can’t contact referrer to get a request sent through etc., then patient will need to be rescheduled. 

If a patient comes in to the practice to make an appointment, please remember to scan the request into Comrad so that we have a copy if the situation arises whereby a patient does forget their request.

If a patient is referred from a hospital (please check all Obstetric patients), it may be worthwhile checking if the referring Drs details and Drs provider number is written clearly on the request and if not clear, and if patient doesn’t know, they will need to find out who their Dr is as well as their provider number so that when patient comes for their appointment we don’t waste time trying to determine who the referring Dr at the hospital is.

The Consultant Dr Hospital list is current except for Concord and Hawkesbury Consultants. If requests are received from these hospitals then the individual referrer Dr details will need to be obtained.
