Nuchal Worksheets

A reminder that all nuchal worksheets need to be checked to ensure that all information required has been added, they are also to be checked against what has been written on the pathology referral to make sure the information matches. This is to be done before BOTH worksheet and pathology referral are scanned in. 

If you are at a site where there is only one sonographer, please have the administration staff cross reference the information. 

Far too many worksheets are being scanned in with missing information or discrepancies between the worksheet and the pathology request. 

If changes are required for the pathology results, it will be the responsibility of the sonographer to call the lab and have the results corrected. 

The checked by initials on the worksheet are for the person who has checked the worksheet and pathology referral, not for the sonographer who has checked the films.
