If a patient wishes to have their scan done under workers comp etc. - all info must be completed and it is up to the person attending to the patient to make sure the info has been completed in full. Example - only noting down an employer's name without providing additional info such as contact phone number, address or even an email address is not sufficient information to allow for us to invoice the employer (if indeed the patient requires the invoice to go to the employer). If we already have the approval then this assists but if there is no approval ALL details must be completed to enable us to invoice correctly and receive payment promptly.
